The First Visit — When?
The gold standard recommendation: the first dental visit at age 12 months, or within 6 months of the first tooth eruption. Not at age 6, as many Saudi parents believe.
The reason: 60% of Saudi children experience tooth decay before age 6 (Ministry of Health statistics, 2023). An early visit catches problems before they require painful and expensive treatment, and trains the child to be comfortable in the medical environment without fear.
Parent’s Checklist — by Age
0 to 12 months — Before the Teeth Erupt
- Wipe the infant’s gums with a clean, moistened cloth after every feed (especially nighttime feeds)
- Do not let the infant fall asleep with the bottle in the mouth — sugar lingers for hours and causes severe “baby bottle decay”
- Avoid dipping the pacifier in honey or sugar
- When the first tooth appears (typically 6 to 8 months): start cleaning it with a soft silicone finger brush without toothpaste
1 to 3 Years — The Critical Years
- First dentist visit: at 12 months, then every 6 months
- Toothpaste: a very small amount (rice-grain size) of children’s toothpaste containing 1000 ppm fluoride, twice daily
- Brushing: the parent brushes for the child — children cannot brush adequately before age 7
- Wean off the night bottle: before age 18 months to prevent classic baby bottle decay
- Avoid frequent sticky sweets: the single occurrence is not the problem — the repetition throughout the day is
3 to 6 Years — Full Primary Dentition
- Checkup visit: every 6 months without exception
- Fluoride application: provided by the dentist every 6 months, reduces decay by 30%
- Pit and fissure sealants (Pit and Fissure Sealants): placed on the permanent posterior teeth as they erupt (age 5 to 6 for the first molars), cost SAR 250 to 400 per tooth and prevent 80% of posterior tooth decay
- Toothpaste: a pea-sized amount, 1000 to 1450 ppm fluoride
- Skill development: the child starts learning to brush independently, but the parent still finishes the job
6 to 9 Years — The Mixed Dentition Phase

- Invisalign First / Interceptive Orthodontics (Phase I): for cases of a constricted jaw or anticipated severe crowding — starts at age 6 to 10 before puberty completes. Cost at AQUA Dent Clinics: SAR 8,000 to 14,000
- Sealants for the new molars: the first permanent molars appear at age 6 to 7
- Switch to an electric toothbrush: more effective than a manual brush for children who get lazy
- Monitoring harmful habits: thumb sucking after age 5, pacifier use, mouth breathing — all may cause orthodontic problems
9 to 12 Years — Permanent Teeth Completed
- Final orthodontic assessment: the dentist decides whether your child needs traditional orthodontics, Invisalign Teen, or no orthodontics
- Fluoride and a deep cleaning annually
- Awareness about food and drinks: carbonated drinks (even diet) erode teeth, sticky sweets cause deep decay
- Sports mouthguard: for every child who plays football, taekwondo, or equestrian sports — prevents tooth fractures
Warning Signs — When to Call the Clinic Immediately
- Persistent tooth pain that wakes the child at night
- Swelling in the face or gum
- A broken tooth after a fall (a large piece or the tooth coming out of place)
- Persistent gum bleeding for no clear reason
- Pus or a foul odor coming from a single tooth
- A primary tooth that fell very early (before age 4 for anterior teeth, before age 8 for posterior teeth) — may need a “space maintainer”
- A permanent tooth that has not erupted at the expected time (more than a year delay)
Handling a Child’s Fear of the Dentist

- Do not use scary words: “needle,” “pain,” “it will not hurt you” (the child hears that word in reverse). Use: “we drink water through a small straw,” “the short nap”
- The familiarization visit first: ask the clinic for a no-treatment session just to get acquainted — the child sits in the chair, plays with the safe instruments, and gets a small gift
- Choose a pediatric dental specialist or a dentist who works with many children: a different patience and a different approach
- Avoid promising sweets as a reward: a contradictory message. Choose a toy, a book, or an activity

Nutrition and Teeth — Grandma’s Golden Rules Have a Scientific Basis
Almost everything your grandmother said about teeth and sweets is scientifically correct:
- Cheese after sweets: correct. Cheese neutralizes acidity in the mouth and protects enamel
- Carbonated drinks dissolve teeth: correct. A pH of 2.5 to 3.5 dissolves enamel (enamel dissolves below pH 5.5). Diet or non-diet does not matter — the acidity is the problem
- Sticky sweets are worse than chocolate: correct. Chocolate melts and is swallowed quickly; sticky sweets (toffee, sugar gum) cling to the teeth for hours
- Dates are nutritious but bad for teeth: correct. Natural sugar, but it sticks aggressively to teeth. Rinse the mouth with water immediately after dates
- Whole fruit beats juice: correct. Orange juice (even fresh) has high acidity and concentrated sugar. The whole orange comes with fiber and less concentrated sugar
Frequently Asked Questions from Saudi Parents
Do I need to treat decay in a primary tooth if it will fall out anyway?
Yes, absolutely. Decay in a primary tooth causes pain for the child, gum inflammation, premature loss of the tooth that creates orthodontic problems later, and bacteria spreading to the permanent teeth. The last primary tooth may stay until age 13.
Is local anesthesia safe for children?
Yes, when used correctly with a dose calculated by the child’s weight. We use Articaine 4% or Lidocaine 2% in quantities less than 0.05 mL/kg.
When does my child need orthodontics?
Evaluation at age 7. Active treatment may begin at 7 to 10 (interceptive orthodontics Phase I, SAR 8,000 to 14,000) or at 11 to 14 (comprehensive orthodontics). Full details in the pediatric orthodontics guide.
Does medical insurance cover pediatric dental treatment?
Usually yes, more than adult treatments. Examinations + cleaning + fillings + extractions are covered in most policies (Bupa, MedGulf, Tawuniya, MedNet, Allianz) at 70 to 100% for medically necessary procedures. Pediatric orthodontics is usually covered at 50 to 80%. AQUA Dent Clinics does not bill insurance directly — we prepare the report for submission.
My child is very afraid, what do I do?
Ask for a familiarization session only (we provide it free for first-time pediatric patients). For severe cases, we offer Nitrous Oxide sedation (laughing gas) — calms the child without loss of consciousness, and the child is back to normal within 5 minutes after it is turned off.
